Cedars-Sinai Medical Center (CSMC) is an 886-bed acute tertiary care level 1 trauma teaching hospital in Los Angeles, California. The Department of Pharmacy Services provides 24-hour services to all patient care areas. The department consists of decentralized satellite pharmacies, a drug use policy center, central hospital pharmacy, 4 outpatient pharmacies, ambulatory care clinics and 3 outpatient cancer centers. Our staff of over 300 includes pharmacists, technicians, residents/interns, and support personnel. The medical center has an electronic health records and has implemented bar code medication administration of medications and infusion pump integration. This evolving area of practice enables pharmacists to become directly involved in patient care, reduce costly readmissions, and improve patient outcomes. At CSMC, we are engaged in a growing number of transitions of care (TOC) initiatives including collaboration with hospitalist physicians, managing CHF patients, post-discharge follow up on high risk patients including those discharged to skilled nursing facilities, medication reconciliation in the emergency room, during hospitalization and in the pre-operative setting. We are also actively engaged in research to demonstrate the value of pharmacists and technicians in this essential aspect of patient care. CSMC is expanding our TOC program and we are recruiting for 1 new full-time pharmacist positions. Responsibilities includes medication reconciliation across the continuum of care, post discharge patient follow up, supervision of transitions of care pharmacy technicians, education of patient and healthcare providers, and participation in departmental transitions of care program enhancement and expansion. The position is primarily Monday through Friday day and evening shifts with additional weekend staffing responsibility on a rotating basis. Summary of Essential Job Duties: The Pharmacist is responsible for the daily organization and supervision of medication management activities in their areas, including but not limited to provision of clinical and drug distribution services and other duties as assigned. The employee performs job duties with sensitivity to the patients' diverse cultural backgrounds. The employee maintains competency skills to identify medication safety concerns in the age related population and institutes additional measures to provide a safe environment with consideration to the developmental changes in the patient population. Educational Requirements: The applicant must be a graduate of an accredited School of Pharmacy. License/Certification/Registration Requirements: Must have a valid Pharmacy License in the state of California BLS from the American Heart Association Experience: Must have completed a pharmacy practice or primary care residency or possess equivalent experience Physical Demands: (e.g., lifting, standing, walking) Physical Requirements (physical abilities essential to perform the job): Frequent standing; walking, bending; reaching, occasional sitting, pushing & pulling, lifting of materials up to 40 pounds for distances to 10 feet. Responsiveness to auditory alarms and communication devices including: telephones, beepers, lire alarms, and mechanical failure alarms, Sufficient mobility to access equipment, patients and other customers. Able to read information on medication labels Work Environment (work environments, hazards and exposures that may be encountered on the job): Exposure to minimal but constant noise levels. Repetitive hand motions, Some functions may include incidental exposure to hazards associated with preparation of hazardous medications. Some functions may require use of personal protective equipment. Use of standard precautions for infection control.
