Description The Supervisor, Care Management leads teams of nurses and behavior health professionals responsible for care management. The Supervisor, Care Management works within thorough, prescribed guidelines and procedures; uses independent judgment requiring analysis of variable factors to solve basic problems; collaborates with management and top professionals/specialists in selection of methods, techniques, and analytical approach. Responsibilities The Supervisor, Care Management oversees the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. Decisions are typically are related to schedule, plans and daily operations. Performs escalated or more complex work of a similar nature, and supervises a group of typically support and technical associates; coordinates and provides day-to-day oversight to associates. Ensures consistency in execution across team. Holds team members accountable for following established policies. Required Qualifications + Active Registered Nurse (RN) license + 3+ years clinical acute care experience + 3 to 5 years combination professional and management experience within the following; team lead, or charge nurse, project, process leadership, or Subject Matter Expert, SME. + Proficiency in analyzing and interpreting data trends. + Progressive business consulting and/or operational leadership experience + Comprehensive knowledge of all Microsoft Office applications, including Word, Excel and PowerPoint + Must be passionate about contributing to an organization focused on continuously improving consumer experiences Additional Requirements Hours: Monday-Friday 8:30-5:00 Travel: Must be willing to travel to an market office location on a quarterly basis at minimum. Role may require 5% travel to meet with members at facilities. WAH/Remote Requirements: + Must have a separate room with a locked door that can be used as a home office to ensure you and your patients have absolute and continuous privacy while you work + Must have accessibility to hardwired high speed internet with minimum speeds of 10Mx1M for a home office (Wireless and Satellite are prohibited) Transportation: + Possession of a valid driver's license and access to a reliable automobile + This role is part of Humanas Driver safety program and therefore requires an individual to have a valid state drivers license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits. TB Screen:: This role is considered patient facing and is a part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB. Preferred Qualifications: + Bachelor's degree or advanced degree in nursing or business health field + Sickle Cell, HIV/AIDS Oncology experience + 5 or more years of previous management/supervisor level experience to include hiring, training, coaching up to termination + Previous experience working in a managed care field + CCM Additional Information: Interview Format As part of our hiring process for this opportunity, we will be using an exciting interviewing technology called Montage Voice to enhance our hiring and decision-making ability.
