Assist patients and families w/medical and social issues related to hospitalization and continuity of care. Services provided include psychosocial assessments, counseling and education, evaluation and support, discharge planning and appropriate reporting mandated by law


Masters Degree in Social Work, must be licensed by State of MN as LICSW and eligible to provide clinical social work supervision to support licensure.


Utilizing critical thinking while applying a bio-psycho-social model as well as knowledge of family systems theories, the medical social worker assists patients and their families with issues related to both hospitalization and continuity of care. The Social Worker:
  • Provides social work services including assessment, planning, and intervention. Patients are identified through predictive tools and referrals from providers, staff, or community caregivers to facilitate transition planning from the hospital when medically indicated by medical staff.
  • Collaborates with the healthcare team and community care providers to ensure that an effective and timely smooth transition in level of care is implemented.
  • Provides patients with education, advocacy, and decision making support related to the impact of medical illness, mental illness, social needs, functional needs, or financial needs on healing, recovery, chronic care, or end of life care.
  • Assures appropriate reporting of vulnerability or suspected abuse as mandated by law.

Provides individual and group clinical social work supervision to LGSW staff who are pursuing LICSW level of licensure by MN Board of Social Work. Ability to provide clinical consultation and direction related to patient rights, ethics, cultural competent care, SW assessment and documentation standards. Provide support and education function of clinical supervision while also maintain a half time active SW caseload.
