Summary
Provides an integrated continuum of care for patients from admission to post-discharge. Screens for discharge planning needs and initiates a plan for community resources and/or placement in appropriate post-discharge facility. Promotes safe discharge, appropriate length of stay, readmission prevention and patient satisfaction.

Qualifications

Minimum Requirements
Education: MSW Required
Experience: 2 years acute care experience preferred
License: LCSW.
Certification: BLS (AMERICAN HEART ASSN)
