This position will support the Emporia area.


Provides psycho-social support to identified patients through collaboration with the health care team. Provides clinical consultation and therapeutic services to identified chronic disease patients and families on an outpatient basis. This includes the professional activity of assisting individuals and families with personal and environmental difficulties to enhance or restore their capacity for social functioning and to create societal conditions favorable to their goals.

Essential Duties and Responsibilities: include the following. Other duties may be assigned.

Works collaboratively with the multidisciplinary team to meet the holistic needs of the patient/family/caregiver

Demonstrates knowledge of Stormont Vail HealthCare system, community and governmental resources and expertise in accessing these for patient/family/caregiver

Utilizes community resources to assist patient to continue life in community or to learn to live within limits of disability

Develops appropriate treatment plan based upon assessment, specific concerns of client/family/caregiver, and recommendations to other treatment providers

Establishes supportive and therapeutic working relationship with patient/family/caregiver

Maintains confidentiality and complies with professional ethics according to professional, departmental, and organizational standards

Actively participates in patient care conferences

Demonstrates excellent communication skills working daily with health care providers

Communicates in a clear and concise manner with patients/family/caregivers regarding patient status and progress. Adjusts communication style responsive to receiver.

Evaluate and manage the practical problems such as those that are illness related as well as concrete needs such as housing, food, financial assistance, education, employment, language and cultural barriers.

Evaluate and manage patient/family/caregivers psychosocial problems including conflict/isolation, treatment decisions, quality-of-life issues and transitions in care, advance directives, domestic violence, coping/communication and functional changes.

Utilization of clinical documentation, statistical reporting and evaluation to improve services, assure quality and develop programs

Maintains a professional level of productivity and coordinates and prioritizes patient related activities in an efficient and effective manner

Demonstrates initiative and provides leadership in the face of changes and/or development of new programs or services

Requires Masters in Social Work (MSW) and one year medical experience or Bachelors in Social Work (BSW) and four years acute care/medical experience. State of Kansas licensure required. Practicum experience or work experience in clinical setting dealing with families preferred.
