Responsibilities

Under the general supervision of the department director, this position serves as a member of an integrated, multidisciplinary team that is focused on the assessment, support and/or post-acute disposition of NWH patients and their families.  This position will seek opportunities to create improvements in resource utilization, length of stay and appropriate discharge planning criteria.  These duties will be performed in accordance with, and in adherence to, the mission, vision, and values of Northwest Hospital.

Qualifications

Education Background

  • Masters degree in Social Work (MSW) required

Licensure, Registration, Certification

  • Requires Washington State Registration. 
  • Prefer Independent Social Worker (LSW) and Washington State License.

Special Training/Skills 

  • Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned unit/department.
  • Ability to flex to assist other department members as needed.

Work Experience 

  • One to two years acute medical experience with discharge planning and resource utilization strongly preferred.
  • Familiarity with Geropsych

Physical Abilities

  • Able to sit for extended periods of time.  Able to operate a computer.
  • Must be able to handle a fluctuating workload
  • This job requires ability to communicate clearly in English (verbal and written) with all stakeholders (staff members, clinicians, practitioners and the general public).

Physical Demand Level:  Sedentary

In an 8-hour workday, must be able to-

Lift / Carry 10 pounds occasionally

Sit, Stand, Walk, Bend, Squat, Kneel, Crawl, Climb and Reach above shoulders

Push and Pull

Perform fine motor functions

Must possess functional vision, hearing, and speech in order to communicate effectively with clients, client advocates, and staff.
