Starting Wage: $31.20-$35.10 with full benefits

Job Objective/ Summary: Collaborates with the nursing staff (charge nurses, med passers, caregivers, restorative aids, and recreational therapy department) to provide, plan, coordinate and manage nursing care and services for assigned residents living in the Health Care Center of Friendsview. Is fully responsible for the care of residents living in Health Care Center (as assigned).

Essential Functions:

  • Assists the Director of Nursing & Health Services & Assistant Director of Nursing in planning, developing and supervising the activities of staff providing health care and nursing services to Health Services residents.

  • Regularly evaluates and assesses health related needs of residents; completing resident assessments as required; collaborates with Charge Nurses and the interdisciplinary team to optimize residents health and modify service plans taking the residents and the residents familys wishes into consideration; ensures the delivery of nursing care according to physicians orders, care plans and established standards and policies to make sure quality care is provided, in accordance with Oregon Administrative Rules and Friendsview Policy.

  • Works closely with Social Services Coordinator/Social Worker to plan multi-disciplinary care conferences on schedule as defined by Friendsview policy and state statutes.

  • Provides training and guidance to direct care staff. Collaborates with the Health Services Leadership Team to plan and deliver staff on-boarding, orientation, and in-service trainings.

  • Provides supervision to RN, LPN, CMA, CNA and Resident Assistants providing health services and employed by Friendsview Retirement Community. Delegates work as appropriate based upon qualifications.

  • Ensures initiation of appropriate preventative and rehabilitative nursing procedures.

  • Acts as point of contact for all medical issues for residents living in the Health Care Center.

  • Works closely with the Health Services Leadership Team, providing information regarding resident service issues and problems.

  • Ensures compliance with relevant state statutes, regulations and licensing requirements for assigned care areas as they pertain to medical and direct care staffing functions.

  • Ensures that medication administration is as ordered, in accordance with nursing standards and organizational policies.

  • Interacts with families, pharmacists, physician office staff, and others in order to ensure that a residents needs are met; promotes good facility public relations during interactions.

  • Investigates resident incidents, identifies safety hazards, and institutes corrective action in a timely manner.

  • Collaborates with the Director of Nursing & Health Services and Assistant Director of Nursing in resolving departmental deficiencies with implemented corrective action plans.

  • Assists in processing and monitoring infection control reports, medication incident reports, and resident incident reports for QAPI.

Leadership & Communication

  • Serves as a member of the Organizational Leadership Team.

  • Provides leadership to Health Services staff in a manner that is consistent with the Friendsview values, promoting teamwork, excellence and integrity in all services provided.

  • Ensures departmental practices maintain high morale and staff retention, to include effective communication, prompt problem resolution, positive supervisory practices, and a positive work environment.

  • Works closely with Health Services Administrative Assistant to assure staffing resources are available. May provide staffing coordinator coverage as needed.

  • Works cooperatively and maintains positive relationships with residents, visitors and staff in all departments.

  • Communicates effectively and respectfully with all staff, residents, visitors, and outside medical and other personnel.

  • Follows up with appropriate staff, residents, or other individuals regarding reported complaints, problems, and concerns.

  • Understands, complies with, and promotes all rules and regulations regarding residents rights; and promotes positive relationships with residents, visitors and regulators.

  • Complies with, supports, and enforces all policies involving safety and infection control procedures to include safe resident handling for employees and residents.

  • Adheres to Friendsviews policies and procedures, including those covering privacy and security in compliance with HIPAA regulations, and codes of conduct. Maintains confidentiality and respects residents privacy at all times in accordance with HIPAA regulations.

Qualifications and Job Requirements:

  • Job Education, Licenses/Certifications, Knowledge & Experience:

    • Registered nurse in good standing, currently licensed in the state of Oregon.

    • Graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.

    • 3-5 years of responsible nursing experience, preferably in long-term care; and minimum 2 years relevant experience in a clinical setting, with management or supervisory experience.

    • Thorough understanding of federal and state rules and regulations pertinent to the organizations health services licensing and licensing commitments.

    • Knowledgeable in the development and use of care plans/service plans, medication management systems, infection control practices, nurse delegation practices, and residents rights.

  • Supervising (direct reports and level of supervision): Normally responsible for full range of supervisory responsibilities for direct service positions/caregiving staff in the respective Health Services neighborhood (Health Care Center, Special Care Center, Residential Care Center), including providing daily work direction, hire, discipline and terminate employees, make pay adjustments, communicate performance appraisals, approve absences, and/or make job/assignments changes subject to department director input and approval.

  • Decision Making: Works with minimal supervision. Decision-making involves weighing complex factors and engaging with/interacting with others to solve problems. Guidance and direction are available upon request. The scope of decision-making responsibilities is such that decisions or judgments have the capacity to adversely impact operations.

  • Interpersonal Skills/Interactions: Communicates regularly with employees, residents, family members, and medical professionals. Interpersonal skills are a significant aspect of the job responsibilities, and skills must be highly refined. Interactions are regularly non-routine, complex, and may require wide-ranging discussion or explanation. Regular interactions with others which may be complex, demanding, difficult or disagreeable. Formal presentations may be frequent.

  • Must demonstrate computer skills necessary to perform all essential functions. . Demonstrated knowledge of/competence with Microsoft Word and other Microsoft Office programs. Know or be able to learn database management programs. Must be able to create reports and analyze data.
