Patient Services Associate - Health Connections

**Summary:**









The Patient Service Associate position is a non-clinical position whose primary responsibility is to the provider and patients. PSAs are responsible for coordination of all transfer calls from referring physicians into Stormont Vail Health and for contacting the appropriate Trauma team in the event of a Full or Partial Trauma alert. PSAs are also responsible for scheduling hospital patient discharge appointments, providing reminder calls for appointments. PSAs maintain current on-call schedules for all Cotton ONeil, Pediatric Care, Sabetha Community Hospital, Clay Co Hospital, Hays Medical Center, and Geary Community Hospital. They also are responsible for maintain call schedules for Special Teams (Cath Lab, EP Lab, Perfusion, Peripheral Angiography and various other pt care groups), Clinical Research and Oncology Radiation staff. The PSA position is accountable to receive and process a high volume of incoming calls from community callers and physicians/staff, in a timely and professional manner and perform outbound calls, which may include satisfaction surveys.

















**Essential Duties and Responsibilities** include the following. Other duties may be assigned.













+ Responsible for hospital patient discharge appointments for both Cotton ONeil and non Cotton ONeil patients. In addition, will provide reminder appointment calls/letters.



+ Responsible for accurately contacting of all Cotton ONeil, Pediatric Care, Sabetha Community Hospital, Clay Co Hospital, Hays Med Center, Geary Community Hospital physicians, Stormont Vail Health Hospitalists, Special Teams (including Cath Lab, Perfusion, Cardiovascular, EP lab), appropriate Trauma Team for Trauma alerts, Trauma on-call and back-up surgeons, Trauma Medical Director, ER staff Trauma Nurse Specialist and air transport as needed for Trauma Transfers. Also responsible for contacting of appropriate physician, house supervisor for any potential transfer from outside physicians/facilities.



+ Insures all patient information entered into EPIC is accurate and complete.



+ Answers incoming calls for all the CON Express Care offices; contacts Express Care staff as needed.



+ Responsible for registration of clients of various classes/events sponsored by Stormont Vail Health.



+ Demonstrate ability to accurately document all inquiries into database for statistical purposes.



+ Works collaboratively with personnel in other departments to insure quality service to clients.



+ Maintains a strong communication system within the department to facilitate prompt accurate service to clients.



+ Participates in weekly huddles; utilizes A3 problem solving to improve processes & develop standard work.



+ Ability to accurately enter demographic info, physician, record signs/symptoms of callers needing triage and assign call to Health Connections nursing staff pool within EPIC.



+ Receives incoming telephone calls; screens and appropriately routes calls to other departments and medical staff as appropriate.



+ Demonstrates knowledge of age appropriate protocols regarding transfer and trauma calls



+ Participates in monthly staff meetings



+ Responsible for assigning and notifying SV hospitalists of ER patients who will be admitted.

**Education/Experience:**













+ **High School diploma or general education degree (GED) required.**



+ **Two years customer service experience required**



+ **Two years health care experience preferred**



+ **Two years of college, associate or bachelor degree preferred**



+ **Basic medical terminology course (required within 90 days employment).**



















**Supervisory Responsibilities:**









No supervisory responsibilities.
