**Description** Senior Bridge Private Pay Homecare is seeking an experienced RN to join our team and provide flexible Evening and Weekend On-Call coverage. Ideally looking for 10-15 hours per week, but could grow to more. Weekend coverage will include some home visits. Our professional RN Care Managers are dedicated to delivering the highest quality in-home care management to senior clients and their families. On Call coverage is 5pm - 8:30am on weekdays, and 24 hour coverage on weekends. This is an an opportunity to boost your income and work a flexible schedule. We offer Weekly pay, PTO, 401K benefits, and an opportunity to grow this job into a larger role with SeniorBridge. **Responsibilities** The Home Care /On-Call Nurse mostly provides phone coverage for calls of a clinical nature that occur after hours. There may be some unplanned or scheduled home visits to perform the following: + Initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es); + Serves as case manager in all cases nursing and therapy for which he / she may be conducting assessments and visits; + Responsible for the clinical record for each patient receiving nursing services; + Regularly re-evaluates patient nursing needs; + Initiates the plan of care and makes necessary revisions as patient status and needs change; + Uses health assessment data to determine nursing diagnosis; + Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician; + Counsels the patient and family in meeting nursing and related needs; + Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required. + Communicates with community health related persons to coordinate the care plan. + Instructs, supervises and evaluates home health aide care provided every two (2) weeks. **Required Qualifications** + Registered Nurse in the state of CT + One to two years of recent acute care experience including medical surgical experience + CPR certification + Excellent verbal and written communication skills + Sensitivity to cultural diversity and to making accommodation available to provide services to a diverse population + Ability to work well under pressure; critical thinker + This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program.
