We are seeking a compassionate, dedicated Nurse Practitioner to execute the following job duties with our inpatient Hepatobiliary/Liver Transplant population. **Essential Duties:** + As the Nurse Practitioner, you will be responsible for the planning and coordination of all aspects of the work up and care for the patients in the Hepatology Program, throughout the continuum of services - from initial referral to follow up care. + You will be responsible for evaluating the patients (includes: chart review, performing history and physical examinations, ordering diagnostic tests, reviewing results and reporting to the Physician as needed. + You will provide Inpatient services to Hepato-pancreatico-biliary (HPB) and Liver Transplant patients, coordinate daily activities of inpatient service, including facilitation of early discharge and care coordination/patient education, timely documentation of patient progress notes, discharge summaries, and CMS required care coordination notes. + You will coordinate communication of patient ADT and surgeries with referring and collaborating MDs. + You will develop and implement patient management plans and assist in provision of continuity of care. + You will provide education, leadership and patient care management across the health care continuum from preventative and acute care through discharge. + The Nurse Practitioner will serve as a patient advocate throughout the care process and represent the medical center at regional and national meetings/conferences. **Education:** + Masters of Nursing Degree(MSN) required + Graduate of an accredited NursePractitioner program required **License/Certification:** + Current State of CaliforniaRegistered Nurse and Nurse Practitioner License required + National certification oreligibility for certification as an NP required + Current ACLS certification fromthe American Heart Association required + DEA license required (oreligibility to obtain) **Experience:** + A minimum of 2 years of acutecare nursing experience required + Experience as a NursePractitioner preferred + Experience with Hepatologypatients desired \#Jobs-Indeed \#Jobs-Glassdoor \#Jobs-LinkedIn \#Jobs-NHCN \#Jobs-HERC Cedars-Sinai is an EEO employer.
