Manager- Oncology Care Coordination

Job Summary:

The Care Coordination Team Manager is a serving leader who is a clinical and operational link between clinical care strategy and the Clinical Care Coordinators, ensuring that there is clinical standardization and programmatic consistency throughout all locations. Interfaces with clinical care coordination team regularly and serves as the point of contact for Care Coordination activities. This position works with multiple entities to include nursing, market and network services, payer contacts, information technology, primary care and specialty care leadership, enterprise care coordination and other disciplines, as appropriate. Functions as a clinical care coordinator in coverage situations and ensures best practices across the program.



  • Monitors Care Coordination team's core competencies, clinical and utilization outcomes, quality metrics, outreach compliance, and ensures appropriate clinical intervention design.
  • Serves as the liaison between various payer programs, clinical care practices, market and network services, and the care coordination teams.
  • Is an expert and resource to all care coordinators regarding best practices, protocols, workflows, and program changes.
  • Acts as the key care coordination information and education resource for the interdisciplinary care team.
  • Maintains procedure manual and communicates changes to care coordinator in collaboration with physician staff partner.
  • Collaborates with clinical leadership on protocol review and changes.
  • Ensure workflow alignment with enterprise care coordination and payer’s quality and programmatic agreements.
  • Conduit for ongoing communication.
  • Works collaboratively with a multidisciplinary care team across the continuum of care to provide care coordination coverage to high and rising risk patients when front line care coordinators are out of the office.
  • Primary responsibilities include ensuring that care coordinators are identifying, assessing, educating, and coordinating care for the high and rising risk patients identified in the clinical setting with a focus on patient outreach and the goal of achieving optimal outcomes.
  • Ensures appropriate distribution of patient load and effective management of patient panel.
  • Provides leadership in hiring and onboarding of new nurses in collaboration with their respective practice and interdisciplinary team.
  • Serves as a coach and mentor for all clinical care coordinators, contributes to and completes APRs.
  • Other duties as assigned


    • BSN required.
    • MSN or Master's in Healthcare related field preferred.


      • Current state licensure as a Registered Nurse (RN).
      • Basic Cardiac Life Support (BLS) required.

        Work Experience:

        • Five years of nursing experience required with one year experience in Care Coordination or case management.
        • Previous supervisory or management.

          Complexity of Work:

          • Excellent interpersonal communication capabilities.
          • Demonstrated leadership skills.
          • Strong analytical, process, data management and PC skills.
          • Strong organizational, time management and quality improvement skills.
          • Ability to work with diverse groups.

            Physical Requirements:

            • Requires direct patient care.
            • Requires extensive walking, telephone use, bending, reaching, and lifting items weighing twenty pounds or less.
            • Must be able to work well under pressure and maintain professional demeanor under adverse conditions.
            • Extensive reading, writing, use of pagers, fax machines, computers, and copiers.
            • Sedentary Work - Exerting up to10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body.
            • Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time.
            • Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
