Under the direction of the RN / Mid-level Provider / Physician and in collaboration with other members of the health care team, contributes to the assessment, planning, implementation, and evaluation of patient care.
Basic Qualifications:
Education - High School Diploma or equivalent Associate's Degree in Graduate of an approved nursing program
Experience - Successful completion of competency based orientation program
Licenses -
Licensed Practical Nurse in MI (LPN)
Drivers License – MI (MI DR LIC) NRS Home Care, VNA, and Hospice
Basic Life Support (BLS) Medical Group, Pennock, Spectrum Health Grand Rapids, Gerber, Continuing Care (except Hospice), Ludington, Reed City, United, Zeeland 90 Days
Brain Injury Specialist, Certified (CBIS) Continuing Care 2 Yrs
Neonatal Resuscitation (NRP) Spectrum Health Grand Rapids Neonatal Services, modules 5-9 90 Days
Preferred Qualifications:
