Cherry Health is a federally qualified health center with a mission to improve the health and wellness of individuals by providing comprehensive primary and behavioral health care while encouraging access by those who are underserved. Working in an integrated team model, the LPN provides basic nursing care and performs supportive clerical tasks required to deliver primary care services to patients. A desire to work with an underserved patient population will be important. This is a full-time, benefit-eligible position working 40 hours per week. The schedule is typically Monday through Friday between the hours of 8 a.m. and 5 p.m., however some evenings will be required.

Requirements:



  • LPN license in the state of Michigan

  • Basic Life Support/CPR certification 

  • Bilingual in English/Spanish preferred

  • One-year experience as an LPN with ambulatory care experience preferred, or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job



Responsibilities:



  • Perform all delegated duties within the scope of education and validated competencies

  • Assist with patient flow and clinical support functions which may include scheduling appointments and managing external referrals

  • Take and record vital signs

  • Prepare for and assist providers with examinations and procedures, and carry out necessary clean-up

  • Obtain lab specimens, order, schedule, and perform tests, and document results

  • Administer medications

  • Complete clinical documentation and record health information

  • Participate in coordination of care which includes clinical data extraction and documentation at the direction of the RN or provider

  • Implement provider orders

  • Complete chart preparation for each patient including review of the population health management system, review and update incomplete orders and referrals, and gather other information that may be required such as Michigan Automated Prescription Services (MAPS)

  • Take phone calls, gather information, and provide information to appropriate staff for direction 

  • Conduct follow-up calls in a timely manner and provide education and instruction within scope of practice.

  • Other duties as assigned
