Implement social service policies & procedures to meet residents- needs in compliance w/ federal, state & local regulations. Participate in family & resident activities designed to promote social interaction, reality orientation & intellectual stimulation. Help residents & families to deal w/ feelings about death or dying & other emotional, mental, environment or physical limitations.

Essential Functions:
- Assess & complete the sections of the (MDS) Minimum Data Set assigned completely, accurately & in a timely manner
- Document the social service component of the Comprehensive Care Plan for each resident in a timely manner
- Document psycho-social needs interaction & follow-up action(s) w/ resident &/or family according to facility policies & procedures
- In coordination w/, & at the direction of the facilities interdisciplinary team, review & document residents- discharge needs in accordance w/ facility discharge planning policy, & regulatory requirements, & residents financial status
- Coordinate provision of post-discharge home health, long-term care, assisted living, & other services, as required
- Coordinate ordering, delivery, & tracking of required post-discharge equipment
- Verify that residents who are to be discharged have orders for discharge written in their medical record
- Document residents discharge plans, & residents involvement in their plan of discharge
- Coordinate provision of educational materials related to post-discharge care
- Participate in the facilities weekly discharge-planning meetings
- Maintain frequent written & verbal communications w/ KP Health Plan Patient Care Coordinators (PCC5)
- Initiate & maintain written records of post-discharge follow-up telephone calls
- Act as a liaison w/ social, health & community agencies
- Refer residents to social, health & community agencies & complete accurate documentation in residents- records concerning the results.
- Coordinate family & community resources as needed to solve financial needs & to promote emotional security
- Take part in facility in-service programs as required
- Explain residents- rights to residents & residents- representatives
- Participate in admission of residents as assigned, including tours prior to admission
- Plan, schedule & implement room visits for residents unable to leave their rooms
- Participate in the theft loss program
- Cooperate w/ the activity department to plan & conduct events to meet socialization needs of the residents
- Observe all faculty safety policies & procedures
- Accept assigned duties in a cooperative manner
- Perform all duties as directed by his/her supervisor
- Perform all duties assigned in an effective, professional & timely manner
- Observe infection control procedures
- Follow Residents- Rights policies at all times
- Be responsible for the safety of residents under his/her supervision
- Participate in implementation of programs to improve resident care related to Quality Indicators
- Assume Quality Assurance duties as assigned
- Observe all facility policies & procedures
- Assume accountability for all data contained in employees- handbook
- Come to work in neat, clean attire & consistently present an appropriate professional appearance
- Consistently work cooperatively w/ residents, residents- representatives, facility staff, physicians, consultants & ancillary service providers
