Home Care CNA


Functions under the direction of a Case Manager or Nursing Supervisor in providing direct patient care services as outlined below.  Employee may be required to lift, transfer, and transport residents.  Delivers personal care to residents and keeps records on personal health care activities as defined within the respective regulatory environment (State Home Health regulations).

Essential Job Duties:

  1. Under the direction of the Home Care Administrator or Nursing Supervisor the following personal care duties may be performed in assisting the residents:

    1. Bathroom assistance

    2. Bed Making

    3. Breakfast, lunch and/or dinner assistance

    4. Foot care

    5. Support stocking assistance

    6. Laundry

    7. Wake up service

    8. Dog walking

  2. Encourages and offers fluids to provide adequate oral intake.

  3. Observes appearance and behavioral changes in the resident and reports to the Supervisor.

  4. Follows infection control standard precautions and safety policies per agency, state and federal regulations. Uses personal protective equipment as needed.

  5. Maintains clean and neat resident environment. 

  6. May be required to accompany a resident to physician's appointment, shopping or errands. 

  7. Assists residents to meet socialization needs.  Accompanies residents to activities.  Helps residents maintain a dignified quality of life. 

  8. Responds quickly and courteously to requests of the residents and heath care team.

  9. As instructed, provides medication reminders.

  10. Takes and records vital signs as indicated.

Other Duties:

  1. Maintains confidentiality of resident information.

  2. Home Care staff times slips are to be submitted to the scheduler weekly.

  3. Report all incidents/accidents to the home care supervisor immediately.

  4. Follows the direction of the nursing care plan under the guidance of the Case Manager, Nursing Supervisor and/or Home Care Administrator.  Accepts responsibility for completing assignments once it has been accepted.

  5. Responsible for client safety.  Must use care in performing job duties to prevent accident and injury to self and others.  Must be familiar with and use proper lifting techniques.  Maintain compliance with WhiteStones safety program.  If issued safety equipment it must be used.

  6. Utilizes effective written/verbal communication.

  7. Other duties as assigned.

  1. Ability to understand and follow instructions in English, communicate effectively, and perform simple arithmetic. 

  2. Certification as required by State regulations.

  3. Must have minimum 6 months CNA direct care experience.

  4. Current CPR where required by state regulations


Visual/hearing acuity sufficient to understand written/oral communication and to observe the residents condition.  While performing this job, good body mechanics are involved, may be required to lift, transport/transfer clients.  Bending, squatting, standing and walking as required.   Must be able to push approx. 250 pounds in a wheelchair.  Must be able to lift between approximately 50 lbs.  Must be able to work under stress and cope with sometimes difficult, confused, disoriented and physically disabled clients. Good working conditions in the home or the health care center.  Occasionally may be exposed to unpleasant odors, soiled clothing and linen.  May be subjected to verbal abuse or noncooperative residents.
