Health Navigator - Outpatient Chemical Dependency

**Providence is calling a Health Navigator (Outpatient Chemical Dep, Variable Shift, 1.0 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.**
**In this position you will:**
+ Coordinate care for patients before visits, after visits, and between visits and may accompany the primary care provider during the visit, with special focus on patients at higher risk
+ Facilitate scheduling with health providers to assure coordinated continuum of care
+ Assist with the referral process from start to finish, including follow up of incomplete referrals
+ Participate in development of referral tracking workflow
+ Coordinate with other medical facilities. Coordinates care with outside agencies or case managers as pertinent to patient needs
+ Communicate PCP or team information to patients as needed
+ Use Motivational Interviewing concepts to discuss and identify Self Management Goals and develop care plans with high risk patients
+ Educate patients when appropriate regarding healthy lifestyle and their acute or chronic conditions
+ Conduct Risk Assessments for identified populations and discusses strategies to reduce risk with patients based on findings
+ Educate patient on care team, resources, and processes that impact their care
**Preferred qualifications for this position include:**
+ Associate's Degree
+ Experience in an outpatient setting
+ 2 years care coordination experience
+ 2 years Lived experience
