Job Title:  Community Resource Navigator


Reports to:  Access Services Supervisor


Work Location:  Lansing, MI   


Work Schedule:  Monday Friday, 8:00am to 5:00pm, 40 hours per week.


Compensation: $20.38/hr, $42,390/yr plus comprehensive benefits package including 100% employer paid pension, generous time off and 14 paid holidays.  Tri-County Office on Aging is a qualified employer under the Public Student Loan Forgiveness (PSLF) Program.


General Responsibilities:  Under the supervision of the Access Services Supervisor, this culturally and geographically connected individual serves as a link between underserved communities and existing community resources.  Through home visiting and other means of communicating, this individual assists clients in overcoming barriers to health and social services. Position has a major role during times of local, state or national crisis including pandemics, weather and other emergencies.

  1. Addresses the social determinants of health which effect older adults such as lack of access to good nutrition, economic insecurity, unstable housing, social isolation, depression, discrimination including racial, ethnic or sexual orientation.

  2. Advocates for clients and acts as a liaison between clients and other service providers by working with various community organizations including legal services, domestic abuse service providers, Adult Protective Services (APS), community food nutrition organizations and the homeless network.

  3. Works in conjunction with the Millage Services Team members with problem solving and the identification, coordination and delivery of services and supports to older adults.

  4. Plays an active role in the Crisis Services program.  

  5. Member of the TCOA Access services team which includes Information &Assistance and Options Counseling.

  6. Works closely with the Tri-County Office on Aging (TCOA) Supports Coordinators and Home Delivered Meals (HDM) staff.

  7. Provides technical assistance to service providers for developing and implementing plans for assuring the continuation of services in the event of either a disaster or state of emergency.

  8. Develops and maintains sound knowledge base of various relief and assistance programs.

  9. Assesses the needs of older individuals including mental health and cognitive status.

  10. Works with persons suspected of being victims of domestic and elder abuse and coordinates with domestic and elder abuse programs.

  11. Arranges for reassurance and emotional support to older victims.

  12. Provides ongoing support and assistance in returning to normalcy following an emergency or crisis situation.  

  13. Determines client's eligibility for services such as financial assistance, insurance and other programs in place to assist individuals and refers clients to community resources and other agencies to meet identified needs.

  7. Ability to represent the Agency in a professional manner. 

  8. Must be able to adjust priorities to meet deadlines in a timely manner.

  9. Ability to meet department standards with regard to job knowledge, client focus, initiative, productivity, communication, teamwork and attendance.


Minimum Requirements:

  • Associates Degree in Human Services or related field is preferred.  

  • Community Health Worker certification preferred.

  • A minimum of one-year experience in the field of aging is required.

  • Must possess a current Michigan drivers license in good standing (less than 4 point preferred).


Working Conditions:

  • The work environment varies.  When in the office, safe work practices in regard to office equipment, avoiding trips/falls and fire regulations are required.  When traveling to or spending time at residential or medical facilities, exposure to unusual elements such as smoke, unpleasant odors, loud noises and extreme temperatures increases. 

  • Physical mobility is required for sitting, walking, standing, bending and lifting/holding/carrying objects of up to 20 pounds.  Must be able to move around in multiple locations with varying physical environments & barriers.  Must be able to drive.  Ability to enter and access information using a computer.  Must be able to communicate effectively with clients, co-workers and vendors in person and over the telephone. 

  • Sensory requirements include exposure to varying temperatures, noise levels, environments and activities. 

  • Mental requirements include the ability to handle varying and intense levels of stress.
