Description The Manager, Care Management Support contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Manager, Care Management Support works within specific guidelines and procedures; applies advanced technical knowledge to solve moderately complex problems; receives assignments in the form of objectives and determines approach, resources, schedules and goals. Responsibilities The Community Health Worker Program Manager are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area. Manages a team of Community Health Workers Requires cross departmental collaboration, and conducts briefings and area meetings; maintains frequent contact with other managers across the department. + Build and maintain relationships with providers and community resources to support enrollee identification and referrals + Implement community assessments to identify community resource gaps + Work collaboratively with other Humana associates as a member of Humanas Comprehensive Care Support team, including case managers, peer support specialists, housing specialists, and SDOH coordinators + Serve as a key knowledge source for community services and information for enrollees and other Humana associates + Support and maintain key Medicaid programs Required Qualifications + Bachelor's Degree or higher + 6+ years of experience with Community Health initiatives or Case Management + 2+ years of management experience + Demonstrated ability to conduct briefings and area meetings using excellent organizational and communication + Native or long time resident of Louisiana and involved in the community + Able to travel throughout LA + This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits Preferred Qualifications + Master's Degree or higher + Applicable State licensure in relevant field + Familiarity with state Medicaid guidelines + Proven knowledge and understanding of eliminating barriers to care for Medicaid populations + Previous experience working with Medicaid, TANF, and CHIP populations Scheduled Weekly Hours 40 About Us Mission: At Humana, our cultural foundation is aligned to helping members achieve their best health by delivering personalized, simplified, whole-person healthcare experiences.
