The inpatient Clinical Social Worker LCSW position is responsible for the development, planning, implementation and evaluation of all social service-related activities for the Cedars-Sinai Medical Network Services. The position demands for an intensive focus on crisis intervention and counseling, problem-solving and conflict resolution, patient and family management, interdisciplinary collaboration, biopsychosocial assessments, education, advocacy, and community resource linkages. The LCSW is available to physicians, nurses, case managers, and other members of the care team in the treatment of individuals being admitted through the Inpatient Specialty Program. These individuals include, but are not limited to, high risk and frail patients, seniors, and those with chronic and terminal medical conditions. CSMC services individuals from various health plans including Managed Care, Medicare, Medi-Cal, even uninsured. ESSENTIAL JOB DUTIES: Patient-Focused + Performdirect client interaction and complete a biopsychosocial-spiritualassessment and make recommendations that will assist in the delivery ofthe care plan. + Providepsychosocial interventions that would enhance patients well-being andrecovery such as supportive listening and counseling, motivationalinterviewing, encouragement and guidance, strengths perspective,solution-focused, crisis intervention, problem-solving, education, advocacyand linkages. + Addressand assist in the resolution of psychosocial issues that are hinderingpatients care such as substance abuse, noncompliance, lack of insurance,dysfunctional family dynamics, mental health, financial problems,healthcare illiteracy, unemployment, adjustment to and coping withdiagnoses, lack of transportation, abuse/neglect,disability Family/Social System-Focused + Providesupportive interventions that focus on family management including, butnot limited to, problem-solving, crisis intervention, conflict resolution,and bereavement. + Coordinatefamily meetings and participate in end-of-life, goals of care, andcompliance discussions. + Identifyor connect patient to available support systems that will help the patientin his/her time of recovery and to remain independent and safe at home. + Educatepatients and their family about Advance Healthcare Directives and POLST. Community + Mobilizeavailable community resources to patients such as cancer resources,support groups, free/low-cost health clinics, AIDS/HIV resources, Meals onWheels, AA/NA/EA, Alzheimers Association, transportation + Leadin the proper reporting of suspected abuse / neglect and othergovernment-related reporting such as Child Protective Services, AdultProtective Services, Office of the Public Guardian, Long-Term CareOmbudsman, Community Care Licensing and domestic violence through lawenforcement agency. + Representas a liaison to community agencies and educate them in navigating theseresources independently such social services from the Department of PublicSocial Services including Medi-Cal, SSI, General Relief, CalFresh, In-HomeSupportive Services + Clarifycommunication barriers between the patient, family and the care teamespecially involving the understanding of hospice care, palliative care,skilled nursing facility, home health and other levels of care. Care Team + Attendthe ISP team rounds and participate in interdisciplinary collaborationswhich include physicians (hospitalists, primary care physicians andspecialists), nurses, case managers, pharmacy, administration, andspiritual care. + Actas a consult to the care team in addressing psychosocial barriers totreatment and discharge planning. + Makerecommendations that would advocate for the best interest of thepatient. + Communicatewith the care team especially the referral source regarding findings,recommendations, and outcomes. + Assistin resolving barriers in complex discharge planning. + Supportthe care team in assessing psychiatric crisis such as 5150 and referral tothe Department of Psychiatry and PET, and assist in placement forpsychiatric hold. Administrative + Documentpatient encounter and interventions that assisted in the delivery ofpatient care on CS-Link and other appropriate case managementdocumentation system. + Developa case plan that identifies long-term goals and short-term goals withobjectives and timeframe, implement, follow-up and evaluate outcomes, andmodify goals based on outcomes or changes in influencing factors oravailable resources. + Reviewcaseloads and record interventions that were performed, compile andpresent statistical outcomes based on the intervention and identifyfindings to incorporate in social work practice. + Developresource hand-outs and education materials to patients/families. + Participatein Inpatient Clinical Workgroup and other committees in the evaluation ofinpatient/outpatient case management/social work practices and assist indeveloping and implementing new or revised workflows, policies andprocedures. + Developand maintain a resource directory that is available to the Department ofCase Management and physicians offices. **Education:** Masters degree in Social Work/Social Welfare required. **License/Certifications:** LCSW licensure required. **Experience:** Two to three years of work experience in clinical social work, healthcare social work, and case management preferred. Candidate should also have a knowledge of community resources. Must possess excellent assessment, writing and communication skills. **Working Days/Hours:** Day shift. Monday-Saturday as needed.
