Job Summary

The RN care manager is an exciting and growing RN role that allows the ambulatory RN to provide holistic, evidenced-based care to patients throughout the continuum of care.  Care coordination and shared decision making between RN care managers and patients has been shown to successfully improve patient health and quality of life.   Through this unique role, the RN has the ability to optimize health outcomes over time through advocacy, relationship building and consistent assessment, planning and evaluation of patients needs and progress as it relates to specific disease processes.  

Through both direct and indirect care, the RN will proactively assess, respond to and evaluate the needs of gastroenterology patients, including high risk patients through complex care coordination, nursing interventions, trust building, patient education, motivational interviewing, coaching and mutual goal-setting.
The RN will provide direct care to patients utilizing face to face RN/patient and RN/MD/patient clinic visits as well as essential indirect, non-visit care.
The RN will work closely with providers on a regular basis through team huddles and care conferences to discuss patient care plans.
Face to face visits may include nursing assessment or evaluation, medication and/or treatment administration, patient education, coaching and goal setting.
Non-visit care includes, but is not limited to, telephonic nursing assessment and/or evaluation of both acute and long term needs, patient education, medication management, follow-up care, complex care coordination both within and outside UWMC, post hospital or procedure follow up, outcome management and phone triage.
The RN provides leadership and guidance to the interdisciplinary team to appropriately direct care and delegate care tasks.
The RN will facilitate flow of information, serving as the primary point of contact between multidisciplinary teams, outside facilities and involved family members.
The RN will demonstrate expert critical thinking, time-management and multi-tasking skills as well as the ability to function autonomously and with flexibility in an unpredictable environment.
The RN will identify barriers to care, such as co-morbidities, disease complications, lifestyle and psychosocial issues and will adjust care plans accordingly to ensure optimal wellness.
The RN will play an integral role in the design and implementation of multidisciplinary care plans, algorithms and pathways for specific disease processes and populations.
The RN will utilize the nursing process to evaluate patients readiness to transition out of care management when the patient has demonstrated ability to successfully manage disease or, if needed, revise the care plan as necessary involving the provider and other necessary disciplines.
The RN will serve as the primary advocate and resource for patients within departments at UWMC as well as outside facilities, care partners and services.
The RN will participate in preceptorship, mentorship and education of members of the health care team.


Requirements:   Legal Requirement:
Current license to practice as a registered nurse in the State of Washington. 
2 years med-surg exp. BSN strongly preferred.
