Chronic Care Strategy and Operations Lead

CVS Caremark
Woonsocket, RI 02895
**Job Description**

CVS Health is a Fortune 8 company ($195B in revenue) most commonly known as the largest retail pharmacy company in the U.S. with 10,000 retail locations nationwide. However, CVS is on a journey to become a healthcare company that can make an impact in an industry where change and innovation is greatly needed (healthcare spending to reach 19% of GDP in the next decade, insurance premiums rising faster than wages, shortage of primary care physicians, and one-third of diabetes cases undiagnosed).

CVS has made multiple appearances on the Fast Company list of most innovative companies and is known for bold moves such as discontinuing tobacco sales in 2014, when the company also changed its name to CVS Health.
