BASIC FUNCTION:    
To coordinate tele-psychiatry services which includes: providing education/support to caregivers using the telepsychiatry consult and liaison services and ensuring that those patients who have been assessed and identified as needing ongoing behavioral health care after using telepsychiatry are linked with the appropriate referrals.


NATURE AND SCOPE:    
The Tele-Psychiatry Care Coordinator will provide support to the consult and liaison services provided by the tele-psychiatry consult service, will ensure that caregivers using this service are trained and supported and ensure that individuals that have accessed tele-psychiatry and need follow up care are connected with appropriate referrals.  The position requires daily internal communication and interaction with all team members, management and medical staff as well as frequent external communication and interaction with patients and families.

PRINCIPAL ACCOUNTABILITIES:    
Responsible for working in a safe and protective manner at all times keeping in mind that safety and environmental hazards are the responsibility of all employees for themselves, other staff members and patients.

Provides initial support when a new site begins using the tele-psychiatry service by ensuring caregivers are trained on the equipment and process to use the consult service.

Provide ongoing support to sites using tele-psychiatry.

Coordinates care with both internal and community resources when an individual has received a tele-psychiatry consult and needs ongoing care.

Provides follow up calls, as needed, to individuals who have received a tele-psychiatry consult to ensure they have accessed services.

Keep ongoing records of the number and type of tele-psychiatry consults and the outcomes of these consults.

Performs other duties as assigned.

Demonstrates the ability to provide emotional support to patients and their families.

Demonstrates the ability to clearly and accurately document interventions using local communication equipment, e.g., written, computerized and audiovisual support.  Shares patient’s status reports with ordering primary care physicians and consulting tele-psychiatrists

Demonstrates the ability to communicate clearly and effectively with other members of the multidisciplinary team in varying situations.

Ability to maintain collaborative and effective working relationships with physicians and healthcare professionals.

 

MINIMUM REQUIREMENTS:    
Bachelor’s Degree in a human services related field.

Must be able to complete crisis prevention intervention (CPI) or equivalent.

BLS Certification required from American Heart Association or American Red Cross.
