Join Harborview Medical Center's Continuity of Care team! They have a 100% FTE, Day Shift position available for their RN3 - Assistant Nurse Manager position. (Shifts: 50% will be 10hr, Day shifts, on Saturday & Sunday & 50%, will be Day shift as assigned.)

SUMMARY:
Under the oversight of the CCN Nurse Manager and the Care Management Director, the ANM will provide leadership, coordination, and education on effective early discharge process. The ANM is the primary point of contact and coordinator for the CCN Program and the discharge process on weekends. The Manger will monitor if care coordination tasks are implemented to successfully transition patients from HMC to the patient destination.

The Continuity of Care Nurses (CCN) ANM coordinates all aspects of care leading to a timely The ANM leads the CCN team and also  serves as a liaison for patients and families with medical teams, nursing staff, social services, interdisciplinary team members, consulting and referring physicians and agencies involved in the care of patients from admission through the discharge process. The CCN ANM  in conjunction with the patient and interdisciplinary care team identifies the clinical targets and milestones for discharge and eliminates barriers as needed.

REQUIREMENTS:
Minimum Requirements:
1. Graduate of an accredited school of nursing, BSN required.
2. Current license to practice as a Registered Nurse in the State of Washington.
3. Minimum of three years professional nursing experience in Trauma/General Surgery, Critical and/or Acute Care, Emergency Care, or equivalent education and experience.
Desired Qualifications:
4. In order to lead the team a Case Management or discharge planning experience is highly recommended
5. Effective interpersonal skills, leadership skills, and clinical expertise in the care of acute care patients and care mgmt.
6. Have computer skill to facilitate and document timely interdisciplinary huddle report.
7. Very well developed Communication skill  is critical for the workflow
