The HMC Pharmacy is looking for an outstanding FULL-TIME AMBULATORY PHARMACY CLINICAL MANAGER to join our Pharmacy team! The Ambulatory Pharmacy Clinical Manager is responsible for the implementation and evaluation of clinical services provided by the ambulatory pharmacy.  The Ambulatory Pharmacy Clinical Manager will function as a working leader in providing direct supervision for the ambulatory clinical pharmacists and performs specific leadership functions for clinical services related to clinical practice, education and standards of pharmaceutical care.

RESPONSIBILITIES:

Management:
• Oversee the daily activities of the ambulatory clinical pharmacists.
• Develop and maintain practice standards for ambulatory clinical pharmacist services.
• Develop and maintain indices of clinical competence for ambulatory clinical pharmacists.
• Recruit clinical pharmacists for the ambulatory pharmacy in coordination with the Ambulatory Pharmacy Operations Manager.
• Ensure new pharmacists are appropriately oriented and trained in coordination with the Ambulatory Pharmacy Operations Manager.
• Work with Ambulatory Pharmacy Operations Manager to ensure appropriate scheduling of ambulatory clinical pharmacists.
• Perform personnel performance reviews for ambulatory clinical pharmacists.
• Oversee clinical pharmacist staff professional/career development opportunities.
• Assist with development and maintenance of the staff recognition program.
• Participate in department and organizational quality improvement initiatives.
• Ensure Collaborative Drug Therapy Agreements are reviewed, submitted, and approved as required by the Washington State Pharmacy Quality Assurance Commission.   Perform required quality assurance of CDTA agreements as required by the Commission.
• Provide reports related to clinical practice activities as required by other pharmacy administrative team members.    
• Assist with strategic planning for drug shortages with pharmacy purchasing staff and the Ambulatory Operations Manager and communicate plans to all clinical staff as appropriate.
• Maintain effective communication with staff of the pharmacy, other medical center departments, and the School of Pharmacy.
• Ensure that protocols and guidelines regarding medications are clinically appropriate.
• Serve as a member of committees as assigned.
• Participate in development, implementation and review of departmental policies.
• Assure compliance with departmental and organizational policies and procedures, and state and federal laws and regulations.
• Assume other responsibilities in the management of the Pharmacy Department as delegated by the Pharmacy Director .

Clinical:
• Recommend appropriate pharmaceutical prescription based on patient parameters and interpretation of current literature, safety, efficacy and cost.
• Advise prescribers on patients’ medication therapy.
• Assist in the Pharmacy and Therapeutics Committee process for review and evaluation of treatment options.
• Maintain accurate medication histories and monitors effect and adverse reactions of all assigned patients.
• Participate in multi-disciplinary team activities to provide patient-specific medication information and education when appropriate.
• Evaluate information from prescriber or interviews patients and provides education or assistance for the self-administration of take-home or discharge medications.
• Assist in problem resolution with the healthcare team.
• Direct the work of and instructs pharmacy students, residents, interns and/or pharmacy assistants.
• Develop and present periodic in-services to healthcare providers.

Education/Research:
• Coordinate continuing education programs.
• Coordinate scheduling of undergraduate and graduate rotations with the UW School of Pharmacy, residency directors, and clinical pharmacists.
• Provide lectures for the School of Pharmacy in its undergraduate, graduate, and continuing education programs.
• Coordinate and assign appropriate resources to provide in-services to providers, nurses, and others as requested.
• Encourage collaborative research with the Schools of Pharmacy, Medicine, Nursing, and Public Health.

• Perform other duties as assigned.
REQUIREMENTS:
• Doctor of Pharmacy (PharmD).
• Eligible for licensure as a Pharmacist in the State of Washington.
• Graduate degree in pharmacy and an ASHP-accredited Residency are preferred.
• Three to five years of experience as a clinical pharmacist.
• Demonstrated leadership abilities in the area of ambulatory and/or institutional pharmacy.
