The Penn Medicine Hospice Assistant Team Manager is responsible for assisting the Associate Director in the clinical and administrative operations of the hospice team. The Assistant Team Manager is responsible for assisting the Home Hospice Team Managers in day to day operations. S/he will assist staff with scheduling, patient care issues, patient visits, supervisory visits, chart audit activity and will be expected to manage a small case load. In addition, the Assistant Team Manager will participate in conducting performance evaluations and competencies for hospice employees. The Assistant Team Manager will also be expected in participate in orientation and clinical leadership activities including quality/performance improvement and team meeting facilitation. S/he will also have administrative on call duties for Penn Medicine Hospice.¢\tProvides supervisory oversite for Penn Medicine Hospice Nursing Teams. Functions monitored will include, but are not limited to, nursing case management, regulatory compliance, direct clinical care, new product and service development and management of Human Resources related items for RN staff members. ¢\tEstablish improvement priorities and sponsor improvement activities within Penn Medicine Hospice and Penn Medicine Home Health.¢\tProvide active leadership and direction to teams and effectively implement reengineering/workout initiatives.¢\tFoster the continual integration of UPHS research and education activities into clinical and operational processes.¢\tEnsure that all regulatory requirements are met.¢\tEnsure effective utilization of materials, staff and financial resources. ¢\tPerforms duties in accordance with Penn Medicine and entity values, policies, and procedures¢\tOther duties as assigned to support the unit, department, entity, and health system organization

- Bachelor's Degree\tNursing\tAnd\t3+ years\tProgressive nursing experience with demonstrated leadership skills - Required\n- Graduate of an NLN approved school of professional nursing - Required\t\n- Licensed as a professional nurse in the state of Pennsylvania and/or New Jersey depending on geographic location and scope of responsibilities - Required\n- Certified in Hospice and/or Palliative Care - Preferred\n- BLS/CPR, as a healthcare provider as per the American Heart Association - Required\n- Automobile Insurance - Required\n- Valid Driver’s License - Required
