Job Title: Occupational Therapist, FT, Days, 4215 Therapies, Grove - (Job Number: 108162) Job Number: 108162 DescriptionOccupational TherapistJob Code: 3049 Position Summary:Provides Occupational Therapy Services to home care clients, which includes assisting the physician in evaluating level of function, developing plan of care and implementing plan to achieve client goals. Performs other duties as assigned. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. INTEGRIS Health is an Equal Opportunity/Affirmative Action Employer.Essential Functions:* Conducts evaluation, develops and implements plan to achieve optimal level of functioning of the client within the scope of Occupational Therapist Standards. * Works with client/caregiver and physician to develop short and long term goals.Accountability: Reports directly to the Clinical Manager of the Agency on matters regarding Home Care Clients. Communicates with the Care Manager assigned to the Home Care Client.Required Physical Demands: Keyboarding/Dexterity: Occasionally; activity exists up to 1/3 of the time Standing/Walking: Frequently; activity exists from 1/3 to 2/3 of the time Strength (Lift/Carry/Push/Pull): Heavy (Exerting 50 to 100 pounds of force occasionally, or 25 to 50 pounds of force frequently, or 10 to 20 pounds of force constantly to move objects) Talking (Must be able to effectively communicate verbally): Yes Seeing: Yes Hearing: Yes Color Acuity (Must be able to distinguish and identify colors): Yes This position may have additional or varied physical demand and/or respiratory fit test requirements. QualificationsQualifications:* Bachelor's degree from college or university with an accredited program with a degree in Occupational Therapy required. * Current license as Occupational Therapist in the state of Oklahoma required. * Current drivers license issued in the State of Oklahoma, current auto insurance and acceptable driving record, access to reliable transportation.
